When you register for the WEST, you will be asked to answer the following background questions. Providing accurate and complete background information about yourself is important. All the information that you provide will be kept confidential to the extent required by law. The PESB uses the background information to satisfy legal requirements, to conduct research that benefits educator preparation programs, and to help ensure the fairness and quality of the WEST. The information will be analyzed only as group data, not as individual data. It may be used for statistical and research purposes and will not be used, in whole or in part, to make a determination about any individual. These questions are not required for your registration, but you are strongly encouraged to provide responses to them.

If you register online, you will answer the following questions interactively as you register. If you register by U.S. mail, enter your responses to the following questions on the registration form. The question number corresponds to the section of the registration form where you will record your response.

If you are registering by mail, refer to "Institution Codes" for the three-digit code to enter on the registration form. Please note that only institutions marked with an asterisk in these tables may be selected. If you earned a degree at a college or university outside of Washington State and are applying for Washington residency teaching certification, you may have your test results reported directly to the Office of Superintendent of Public Instruction, Office of Certification, by entering code 399 when you register.

Your test results for all tests you take on the test date for which you are registering will be released to the Washington State institution that you indicate in response to this question. The purpose of providing institutions with test results is to give them an opportunity to review their educator preparation programs in light of examinee performance and to make any modifications or changes to the programs that might be indicated as necessary by the test data. If you do not wish your test results to be sent to your educator preparation program, leave this question blank.
